Report from Victoria Hospital
Prince Albert, Saskatchewan, Canada
Attending Physician: Randy Friesen
Admission Date: 2002 July 22

INDICATION FOR PROCEDURE
Survelliance for colon cancer

FINAL DIAGNOSIS
Normal colonoscopy

PROCEDURE
Colonoscopy and biopsy

FINDINGS
This gentleman last had colonoscopy one year ago. At that time, he was found to have polyps. He is now coming in for surveillance. He has had no new GI complaints. He is feeling well. He is continuing to train for his Iron-man triathlon. Digital rectal examination was within normal limits.

COLONOSCOPY FINDINGS
There were no abnormalities noted from the anus to the cecum, except for a couple of very slightly raised reddened areas right at the anastomosis. They probable represent some mild chronic inflammation, but the larger area was biopsied just to be certain. No polyps were seen.

PROCEDURE
Routine video colonoscopy. Tolerated well.

OPINION
I have advised Mr. Serenelli in writing that he should have colonoscopy again in two years.
